A 35-year old wife and mother with a history of chronic depression and anxiety, including prior suicide attempts, as well as abuse of alcohol and prescription medication, overdosed on methadone and died while under the care of two physicians. The physicians prescribed the drug for the patient despite her questionable history with such substances, including a non-lethal methadone overdose one year prior to her death. Daniel M.
